Health, Social Care and Well-being is about improving the health of local people, and making sure that we can get treatment and help when we need it. This means we can learn how to be healthier, and make changes in our own lives to prevent ill health. It also means local organisations that provide treatment and care must work together, to make sure their services are delivered smoothly and have the best results for the people they are helping.
This theme aims to help us lead a full, healthy life and get treatment, support and care as and when required through:
- Developing local health and social care services.
- Helping people to live as active and independently as they are able, by supporting individuals and the people who care for them.
- Promoting healthy living and helping people improve their lifestyles so they can take greater responsibility for their health and well being
- Tackling poverty and other factors that lead to poor health.
- Working with vulnerable children and adults, to help them avoid problems.
The Strategy looks at a range of services from the provision of hospital and social care services to the promotion of healthy lifestyle choices and wider issues such as, housing, education, our environment and crime.
