Reablement is a short-term programme of intensive support and therapy in a person's own home. Support will typical involve input from occupational therapists, physiotherapists, nurses, case managers and trained support workers, to maximise a persons functions and enable them to continue living in their own home, rather than remaining in hospital or being admitted to long term care.
Who is eligible for reablement?
Caerphilly County Borough Council has a 5-year Domiciliary Care Strategy, which has resulted in expansion of reablement services to enable more people to obtain their maximum levels of independence.
- Any resident of the borough who is over 18 years.
- People who have had a health problem.
- People who have an underlying physical/sensory condition that is likely to improve with therapy and support.
- People who require the service for a time limited period.
- People who require a further assessment of their needs.
What kind of help/assistance can I get?
The service you receive depends on your needs. The team will work with you to help you to be as independent as possible this could include any of the activities below:
- Kitchen skills
- Mobility
- Safety at home
- Management of medication
- Access to the wider community including shopping, leisure and education facilities.
- Our main aim is to work with you to help you to become as independent as possible with every day activities.
- Staff from the Reablement team will visit you to assess your needs and to agree goals, which we will work with you towards.
- The team offers a period of ongoing assessment of your needs.
- The programme and your progress are regularly monitored. As you become more independent, the level of support is reduced.
If needed the team works to establish a care plan for ongoing care services should you be eligible.
How long does the service last?
We are a very short-term service and progress towards your goals is different for each individual person, however we would anticipate you would work with us for a maximum period of 6 weeks. However most people will achieve their goals prior to this time and would be discharged earlier than this
When will I be visited?
A member of the team will normally visit you in your own home to agree your goals. Following this agreed plan the day-to-day visits from our support workers can vary in terms of time of day and duration. This variation is due to the short-term nature of the service and our need to be responsive to peoples changing needs as they progress through the service.
What happens when I am discharged from the service?
In many circumstances we hope you have reached your potential and will no longer require support services. Should you require any ongoing support then the team will arrange for a referral to the appropriate agency for you. If you should be assessed as being eligible for ongoing care services then the team will also apply for this on your behalf, based upon your individual strengths and needs.

Will I have to pay for reablement?
Reablement is a short-term free service You may be asked to make a contribution towards the cost of any on going home care support arranged for you by the team. However, the amount you pay will depend on your financial situation and how much care you receive. More information is provided in the leaflet below, however, we will give you more information about charges before we arrange or provide any services for you.
