Person centred planning

Person Centred Planning is a way of supporting people with a learning disability to plan for their future by putting the person at the centre of planning.

Through working together (with the person, their family, friends and other people who provide support) person centred planning aims to make things happen by listening, understanding and taking action to help people make positive changes in their lives.

A report, written by the All Wales Advisory Group for People with Learning Disabilities in 2001 and called "Fulfilling the Promises" recommends that Person Centred Planning should be used to help individuals and their families plan for the future.

Person Centred Planning and Person Centred Approaches enable people who provide support to listen and discover what is important to individuals now and for the future. It empowers individuals and their families to have more choice and independence and enables people to lead more fulfilled lives, increase their social networks and feel included within their own communities.

Caerphilly County Borough Coucnil has a strategy for Person Centred Planning - the aim of the strategy is to help people understand what person centred planning is, the different styles of planning there are and to offer support to individuals and families to start planning. Feedback from Person Centred Plans will also inform the development of services in the future.

Caerphilly County Borough Council aims to develop Person Centred Planning to ensure that all people with learning disabilities have equal access to planning. This will partly be achieved through training individuals, families/ carers and people/ organisations that provide support.

The following courses will be regularly available:

  • Person Centred Planning – an introduction (2 hours)
  • Person Centred Planning Awareness (1 day)
  • Person Centred Thinking (1 day)
  • Person Centred Planning Facilitator (3 days)
  • Portfolio Building Days (4 hours)
