Gwent Needs Mapping Exercise (GNME)

The GNME form is used to help plan housing-related support services for vulnerable people in Gwent and for equality monitoring purposes. It is NOT an application for services and will not be used to meet individual needs.

Please do not complete the form yourself, it should be completed with your Support Worker, Probation Officer or your Housing Advice Officer. If you do not have a Support Worker and you would like more information, please do not hesitate to contact the Supporting People team on 01443 864591.

If a GNME form has already been completed on your behalf, it does not have to be completed again, unless your needs have changed. If your needs have changed, please discuss this with your Support Worker.

When completing the form on behalf of the person you are working with, you will be asked the geographical location (pdf 6k) of their address within the Caerphilly Borough area. We would be grateful if you could complete this section.
