

To provide short term crisis support to those people in Caerphilly county borough area as well as housing information and advice services to people with a severe, long term mental health problems who are homeless, in housing need or crisis, or who are in danger of losing their home.
The service will support people in accessing appropriate housing and in maintaining their tenancies in order to avoid any unnecessary admissions to hospital or care.
The service will seek to identify housing needs among people with mental health problems and then feed such information into service planning mechanisms.
To contribute to a network of housing and support services in the Caerphilly Borough area.
To liaise with and contribute to the work of the locality mental health teams.
To co-operate with Social Services, Health and other agencies in needs assessment processes to ensure that clients have access to appropriate support services in the longer term to avoid further housing problems and crises.
The primary emphasis of the service will be to address the needs of those in crisis and a flexible and developmental approach to the work will be adopted.
