Home Assist and Reablement Team (HART)

Home Assist and Reablement Team is a Caerphilly Local Authority service. H.A.R.T forms part of the Community Resource Team (CRT) which has three main functions.

Home Assist – Care staff provide support to individuals living within the community who require assistance with daily tasks in order to remain as independent as possible within their own home.

Reablement – Staff assist individuals to follow specific reablement support plans with the aim of rehabilitating individuals back to independence.

Emergency Care – Providing a time limited response to individuals and their families within the community that have reached the point of crisis and require immediate care and support.
